Your membership is effective upon receipt of membership materials.

This is NOT insurance nor is it intended to replace insurance. This discount card program contains a 30 day cancellation period. The plan is not insurance coverage and does not meet the minimum creditable coverage requirements under the Affordable Care Act or Massachusetts M.G.L. c. 111M and 956 CMR 5.00. This plan provides discounts at certain healthcare providers for medical services. This plan does not make payments directly to the providers of medical services. The plan member is obligated to pay for all healthcare services but will receive a discount from those healthcare providers who have contracted with the discount plan organization.

Terms & Conditions

Member is defined as primary member, spouse, and all legal dependents.

Providers are subject to change without notice. Programs may vary in some states. Providers and locations may be removed from the network at any time.

This is a discount program only. The program may be cancelled or modified at anytime. You will receive notice if the plan is cancelled or materially modified.

Normal business hours are Monday through Friday, 7:00 am to 7:00 pm and Saturday, 8:00 am to 5:00 pm Central Time.

The discount company will not reimburse or pay any portion of any provider’s fees. These benefits may not be used with any other discount plan or program. Listed or quoted prices are subject to change without notice.

Providers may offer products or services to the public at prices lower than the discounted prices. In such event, members will be charged the lower price.

Savings are based on the provider’s normal fees. Actual savings will vary by location and the services or products purchased.

This discount program is a referral plan, and makes no warranties concerning the quality of care received. Providers are responsible for the professional advice and treatment provided to members.

Keeping member information secure is a top priority at Savvy Benefits. This document outlines our privacy policy and how we protect and use the data you provide to us.

Your Personal Information

The personal information members provide us is collected solely for the purpose of providing members and their dependents (if applicable) with the New Benefits services and benefits. This information is protected by New Benefits by maintaining physical, electronic and procedural safeguards.

Personal information we collect about you comes from one or more following sources: paper application, faxed application, and web application. Personal information collected includes, but is not limited to: name, address, city, state, zip, telephone numbers, social security number, date of birth, gender, dependent name, dependent gender, dependent date of birth, dependent social security number, dependent relationship, beneficiary name, beneficiary date of birth, beneficiary gender, beneficiary relationship, bank account information, or credit card information.

We may disclose any or all of this information based on business need only, to our affiliates, service providers, third party service providers, provider networks and other suppliers of products and services to New Benefits members.

Savvy Benefits will maintain member information even after membership is terminated for any reason. This information will be protected in the same manner as that of an active member. A member may request in writing at any time after discontinuing the membership to remove the information from our file.

Sharing Information with Savvy Benefits affiliates and Service Providers

Savvy Benefits will share member information with its affiliates, related companies and service providers to the extent it is necessary to provide and manage benefits to the member or their dependents. Unless otherwise permitted by law New Benefits will not share with these parties any information it collects that is not needed by these parties to provide the benefits to the member or their dependents. These organizations are not authorized to use member information for any purposes other than for Savvy Benefits business.

Sharing information with Third Parties or Service Providers not affiliated with Savvy Benefits

Savvy Benefits will not share, sell or otherwise disclose to any third parties not affiliated with New Benefits member information collected by New Benefits. This includes other service providers and marketing organizations not directly related to providing New Benefits member services.

Savvy Benefits Commitment to Members.

Keeping our member information secure and using it only as needed for business reasons to provide services to our members is a top priority to New Benefits. We will safeguard, according to strict standards of security and confidentiality, any information provided to us by our members. We will permit only authorized employees who are trained in the proper handling of member information to have access to that information. Employees who violate our Privacy Policies will be subject to our normal disciplinary process.

We will not reveal member information to any external organizations (other than affiliates and service providers mentioned above), unless the member specifically authorizes us to do so or as required by law. We will always maintain control over the confidentiality of our member information. Whenever we hire other organizations to provide support or services to New Benefits, we will require them to conform to our privacy standards and allow us to audit them for compliance. We will attempt to keep member files complete and up to date and accurate. A member can contact our Member Services Department at any time during business hours to correct or update any information. We will not make available information we have collected in connection with or in anticipation of a claim or legal proceeding.
